Several factors are influencing the adoption of premium IOLs in the United States, including the growing wealth and size of the senior population, increasing public awareness of premium cataract surgery, and improvement in IOL technology.

Companies developing advanced premium IOL technologies continue to focus on the US market, despite the cost and time it takes to achieve FDA approval.

Cataract procedures in general are driven by the country’s elderly population, but the premium share of those procedures (defined as those procedures where the patient pays out of pocket for correction of presbyopia, astigmatism, or post-operative adjustment) is made up of a subset of relatively affluent and healthy seniors.

Market Scope expects premium IOL procedures to account for nearly 20 percent of total US lens removal procedures and 60 percent of revenue in 2022. Overall, premium IOL market penetration has increased by 10 percentage points in the US throughout the past decade, but the penetration rate is highly variable across geographies.

The metro areas of New York, Los Angeles, Dallas, and Chicago account for 14 percent of the population over 60 in the US. However, on a per capita share basis, most of the metro areas in Florida stand out for their potential pool of cataract patients, driven largely by the state’s high median age. Fort Collins, Colorado, is a new honorable mention, and Washington, DC, moves up in rank, entering the top five.

Fresno, California, and Fort Collins have notable concentrations of high-volume cataract surgeons. Denver and Hartford, Connecticut, have high concentrations of candidates for premium IOLs. Minneapolis and Omaha, Nebraska, have high premium surgery rates; the premium IOL rate (premium IOLs/total IOLs) varies widely throughout the US.

To understand the potential for cataract treatment more completely, we analyzed historical reimbursement records; population by age, income, and health status; cataract surgery trends; and aggregate facilities and providers by geographic area. Combining these data created a picture of the overall US cataract market opportunity by detailed geography.

As an example, Albuquerque, New Mexico, and Tulsa, Oklahoma, are similar in population size, yet Albuquerque scores much higher (ranked 24th) than Tulsa (ranked 67th). The population of Albuquerque is older, with a median age of 39 versus 37 in Tulsa. Albuquerque also has a higher median income among seniors and 339 thousand residents with clinically significant astigmatism.

Larger metro areas, such as Chicago and Dallas, have a high quantity of high-volume surgeons. Some metros, such as Fresno and Tulsa, have a high percentage of cataract surgeons categorized as high-volume. Boston and Philadelphia have above-average volume and concentrations of cataract surgeons.
